This is a good way to make contact and arrange a new appointment. You have however created a new user ID on the ssite and it would be better to use the old one 'ANDREA MOORHOUSE' as all you existing questionnaire answers etc are stored in that. I presume you have forgotten the password for that one so I'll reset it for you and send you an email. I'll also book a new appointment to see us again - I've found a cancellation on 15th August and there's a letter in the post. JB
