Dear Dr Bland,
I tried and I am unable to make the appointment for 22nd Oct Thursday this week. I am off work for three weeks 1-20 November. Is it by any chance possible for you to see me then?

Thank you. Can I please take the 4th of November? Please let me know what time. Would it be possible to have a phone follow-up with Dr Bland afterwards at some stage, just in case I need steroid injections.

There is a cancellation on the 4th Nov and I could reschedule you for then but Dr Bland will not be here so will not be able to review your results. Otherwise I can book you for the 23rd November in the afternoon. Which would you prefer? EK
