No verfication email received

Hi there,
What action should be taken if no verification email has been sent? I have attempted to register my grandmother on this site, but the verification email has not been received, nor has it gone to the spam folder.
Please also note that when attempting to complete the questionnaire (which we did after attempting to find the verification email first), the questionnaire would not let you get any further than confirming your gender. It basically jumps straight back to the very beginning of the questionnaire.

It's all explained on this page - there is no verification email if you have a Canterbury reference number. JB
