Referral for surgery on my left hand

Hello Dr. Bland,
I came over to the clinic in November for a reassessment of CTS on my left hand. It came out as a level 3, and I had an injection at the time to alleviate the pain. I was also given an option for surgery. The Injection has eased the pain somewhat, but I would like a referral for surgery if possible.

You have a mixture of CTS, Dupuytrens and trigger digit I think so it would probably be best if the left hand was seen by Mr Smith, who I think did the right side surgery. I'll set about arranging it. JB
