I have been referred through by a hospital doctor and have now received my appointment. Having now completed the Self Diagnostic questionnaire do I still need to Email Dr Bland as per the instructions on the home page? It is a little unclear as I already have an appointment

Probably because you haven't told me about it. You seem to have done it 5 months ago so it's probably getting a bit out of date - have your symptoms changed since September?

It's a good idea to sit down with a good cup of tea and read the patient information page of the website which explains in detail how this system works. JB
There is a high enough probability of CTS that we should test you yes - so keep the appointment. JB
