Hello I have completed the questionnaire ID: 224177 69% CTS User ID: 42222
I have already had two steroid injections to my left wrist. I now require another injection or further treatment as discussed at my last visit to the clinic.

It's over a year now since we last saw you so we'd better see if things have changed. I've sent out an apointment with us for 29th June. We need to use the website rather differently though. You already had an account which uses your name as the username and we need to go back to using that one and delete 'proudcub52' - otherwise it will confuse the computers :-) I'll change the password on the old account for you and send you an email with the login details. Once you can log in to that account you click "My CTS" then "My CTS record" then "Take a new severity score test" to update your symptom severity evaluation. JB
