Mr bland
I had surgery on 25/02/2012. It all went well, scar a little longer than Right hand as the Dr explained,my nerve was trapped a little higher than my right hand ?
I am really in a lot of pain in the middle of my palm, the scar has healed really well, but it feels so painful, the only way to describe it , is it feels like the worst tooth ache ever in my hand ! It is a cramp like feeling that travels up my fingers and thumb. I have tried heat from a hot water bottle, and pain killers but it doesn't help, the pain is constant, and it is waking me up at night. Please can you help ?
Thankyou for your time Tina Peattie

That sounds like a local problem in the scar to me - is it tender on pressure? I suspect that Dr Rine might want to see that so I will email him now. I will also see when we might be able to fit you in to a slot where I could take a look at it when I get into the office tomorrow. JB

Dr Bland,
Yes it is really tender when touched, and I cannot use it to open doors etc. As it is so painful.
Thankyou for sorting it out for me, it has really made me feel low.

OK. There's no time to get an appointment letter out but you know where to find me - just turn up at reception as usual. I've also heard from Dr Rine and he will probably be in touch too. JB

Hi Mr Bland,I went to see Dr Rhine? today at Beltinge.
He said he was going to get in touch with you re post op pain etc. He said he was very puzzled and felt he thought it could be pillar pain ? I told him you had tested and scanned it, and the first test appeared to show the reading was not as good as before surgery, did I get than right ? I am feeling a bit low with the discomfort as it doesn't appear to be getting any better, being far worse at night.
I told Dr Rhine that you mentioned hand therapy he said that was his suggestion as well, (although I felt he was not quite sure) He said he would chat to you further to decide what to do, When you scanned my hand did you see any other reason that it is like it is ? Being quite a nervous person I didn't really understand much. He also said sometimes the nerve can be only partly cut ?

I have indeed had a long talk with John Rine about it late this afternoon. Your test results on Monday were marginally worse than the pre-operative ones but this early after surgery I do not think that means very much and the ultrasound examination suggested that the ligament (not the nerve - you do not want to cut that) had been successfully divided. I will be arranging for the hand therapists to see you to see if they can do some work on the scar and tendons and I will probaby be wanting to see you again myself in a few weeks. In the meantime keep me posted on here please. JB

Several further telephone discussions have been had, and email sent. I'll let you know when I get a reply. In the meantime could I ask you to do me a favour please and complete another severity questionnaire online here so that we get the current state of the two hands documented on here now. JB

Mr bland, I am suffering a lot with shooting pains in my wrist and arm, the left hand side of my arm below the thumb and in the centre of my wrist is tender and appears swollen compared to my left arm. It really is annoying, I have it elevated as suggested I am worried that there has been no improvement since seeing you last week. I do feel my thumb is the problem, it sounds very drastic but would it be best to open it up again to actually sort it out ? kind regards. Tina p

I think purely exploratory surgery runs something of a risk of actually making it worse so I would generally try to avoid it unless we thought there was a clear reason for re-exploration. Sit tight for a few days longer as I am waiting for a response from the hand therapists. JB
