Confused

Hi Dr Bland
I came to see you yesterday, and am now quite confused. As yesterday you told me my hands had deffinatly got better ( which i knew) but you said they were at a level 1, So you suggessted having another injection. I have called the primary in care and they have said that you have written that there is no evidence of CTS. So I'm confused. If there is no evidence, then that is great, but surely would you not have told me that yesterday, so that if what is going on with my hands at the moment got worse i would be able to try and find out what it is. Sorry its just that i would like to get to the bottom of it if it starts getting worse, and if its not CTS then at least i would know.

Sorry about that. In fact you hands are what you might call grade 0.5 at present. When I came to write the report the numbers didn't quite make it to the borderline for classification as grade 1, but one very sensitive test is abnormal in each hand and we do know that you had more of an abnormality previously. Thus, although I would not normally make a diagnosis of electrophysiological CTS on the basis of only one sensitive test (because these have a significant false positive rate) in your case I think those abnormalities are significant. I was too rushed on Thursday to put all that in the report. I'm still inclined to re-inject and I've amended the report to reflect that so that the SIPC office can see my thinking. We will send out an updated version to your GP too. JB

I've got an appointment for injections on the 19th.... And my blood tests have come back clear... So im guessing it is the cts affecting me.... I know in august the tests were pretty good... But the symptoms at the mo seemed to def b staying how they were pretty in the first place.... Im kinda wondering... About the op?? Is this a possibility ??

It's certainly a possibility but there is a bit of a queue for surgery at present and it's probably worth injecting again for now. If nothing else a good temporary response to injection is a good prognostic indicator for surgery. JB

Hi
Yeah i guessed there would be a wait ... If its ok with u... I think i would like to take the option.... I do hope that would b ok... The last two injections have been great... But it does seemed to have come back this time with some avengance. And i know that my dad has never looked back after his op....so am thinking that may b it would b more of a long term solution. Thank for listening....and sorry for being such a pain....i will go for the injections and hope to hear from u at some point...

Let me know how they respond to injection - about a week after should be about right - and then we wil try to work out a surgery date. JB

Ok thanku,i really appreciate it... I haven't had a decent night sleep now for about 3 weeks.and its starting to become hard to use my hands in the morning again..cos of the pins and needles. Its becoming a bit of a nightmare exactly how it was last year. Im not sure which side is worse... For some reason.. for the most of the last few weeks it seemed as thou it was my left.but now um not so sure..both sides seem to be starting to affect me just as badly as each other..if that makes sense.
