Difficulty completing questionnaire

I have been trying to fill in the severity questionaire but my given answers are not registering and I am being asked to reply to questions which I have answered. So, going round in maddening circles at the moment and have given up. Anna Witham
You already have a completed an initial questionnaire and cannot do a new one - the only thing you can do is add a new severity questionnaire any you have done two of those already. JB
