Surgery follow-up

I had surgery at the Broomfield/Reculver practice before Christmas which has wholly relieved my symptoms. There was mention in the process of a follow-up questionnaire but this does not appear to have happened. Do I need to do anything for this to happen?
We send them out about 3 months after surgery - yours should be due about now, probably in the next batch or two. I'm glad to hear it has been successful though. JB
